Why Make a Champions of Care Donation?
If you would like to give back to someone who has provided exceptional care or service, please fill out the donation form below and let us know who you’d like to say thank you to!
Lumacare will facilitate all Champion of Care gifts and present the Lumacare staff member or volunteer with a card and pin that lets them know a donation has been made in honour of their act of service.
Please fill out the Additional Comments section with the name of the person you would like to nominate as a Champion of Care, and a personalized message you would like to send to them.
